Dream Smile Dental Clinic doesn't have a contract with a NHS Primary Care Trust and all treatments are provided on a private basis only.
We shall supply printed details of your strategy once your treatment strategy has been agreed with the Dentist. Whether this strategy alters because of clinical or radiographic findings, we discuss this with you and will notify you. Treatment strategies are valid for net 90 days from the date the treatment was prescribed. Please don't hesitate to inquire us, before the appointments for actual treatments are made if there are any points in your treatment strategy which you wish to query.
Specific treatments require completion of a written authorization form. This is before any of these treatments are completed, to be able to explain the treatment, aftercare and any danger to you thoroughly.
Dream Smile Dental Clinic doesn't operate a credit account system and we require fees to be settled at the appointment where treatment is provided.
Where a laboratory fee is incurred by treatment, at least 50% of the total fee is due at the appointment where impressions are taken. Fees for particular treatments like Invisalign and Dental Implants are taken in staged payments.
Where treatment is paid for on finance through a Finance company, please take note the finance agreement must be accepted and signed just before the start of that treatment. Please note that a minimal deposit can also be demanded, and that finance is exclusively available on treatments through?400, not including Emergency Dental Treatment.
If you have funded any of the prices of your treatment and wish to cancel your treatment after signing the agreement forms, please be aware a cancellation fee of up to 15% of the overall finance sum will be charged.
Pearl Dental Clinic doesn't accept any payments.
Fees for treatment where intravenous sedation is contained, must be paid prior to the appointment in order to avoid financial trades, or signatures being demanded, whilst a client continues to be under the effect of the sedative.
Dream Smile Dental Clinic reserves the right to charge time established deposits for booking appointments that are future. Deposits are deductible off the price of treatment.
Dream Smile Dental Clinic reserves the right to make a fee for any debt passed to a debt collection agency.
All the fees implemented for any kind of Dental treatment at the practice are based on; the stuff used, laboratory costs incurred (if applicable), and the clinical time spent. The fees are nonetheless, shown as treatments prescribed on the treatment strategy for your information.
Late cancellation or appointments that are missed:
Dream Smile Dental Clinic reserves the right to bill up to?100 per hour, in the event of a missed appointment or an appointment.
We understand that some patients travel long distances to get to the practice, and in some events being late for appointments can be unavoidable. Please take note that you might be requested to reschedule your appointment if you're more than 10 minutes.
Ceramic Veneers, at Dream Smile Dental Clinic our Lumineers, Implant crowns and Ceramic crowns are ensured for a period of 5 years following placement, so long as the following conditions are met:
The treatment has been fully paid for by the patient and doesn't owe the practice any money for the treatment received.
The Veneers, Lumineers, Crowns & Implant crowns haven't been damaged as a consequence of an accident, excessive grinding or trauma.
All post treatment maintenance recommendations made by our dentists have been followed by the patient.
The patient has attended and routine evaluation every 6 months.
Some treatments may have a guarantee of less than 1 year, and in this scenario you will be notified by your Dentist either verbally or in writing, or both.
It's very important that you simply give a full medical history and details of any medicine you take. Should these change at all, it is essential for you yourself to tell your Dentist. Use of Pictures and Xrays:
Dream Smile Dental Clinic may use pictures and x-rays of your smile and teeth only, for advertising and educational goals on the Dream Smile Dental Clinic site, and on promotional and educational literature. Your name will never be published, and identity WOn't ever be revealed. But if you DO NOT wish for us to use your pictures and x-rays in this way, please let's know.
Use of patient contact details:
At Dream Smile Dental Clinic the health of our patients is our highest priority, and we also like to keep our patients informed of various changes that are significant at the practice and of our latest special offers. We like to remind other various reminders that are significant, and our patients of their appointments, when they may be due for appointments. On this note, you might be occasionally contacted by the practice via cellphone, text, e-mail or by letter in the post. Please let us know if you DO NOT wish to be contacted by the practice all or by any of these means.
To be able to ensure that every patient has only the best experience at all times, at Dream Smile Dental Clinic we constantly take complaints about any aspects of our services very seriously.
Complaints can be made in writing by filling out a simple complaints form available from reception, or via e-mail through our online site contact form, by the patient or by an authorized person on the patient's benefit.
Every complaint will receive immediate written/e-mail recognition, and the Management will strive to resolve the grievance within an instant, reasonable time period (generally about 2 weeks). For our complete complaints procedure and policy, please request an associate of our reception team. If a grievance is about any aspect of clinical care or associated charges it'll normally be recognized by the Management team and after that referred to the treating Dentist. Please note that in these events, Dream Smile Dental Clinic acts as a liaison between the patient and the treating Dentist, simply and accepts no obligation on behalf of the treating Dentist.
Should you be unsatisfied with our internal handling of your complaint, you happen to be free to forward your complaint to the Dental Complaints Service (www.dentalcomplaints.org.uk). If still unsatisfied, you may wish to forward your complaint to the General Dental Council.
No forbearance/Maltreatment policy
In these scenarios, the right to refuse treatment and entrance is reserved by Dream Smile Dental Clinic.
Data Protection Act:
We save all patient personal details on a secure computer system in accordance with the Data Protection Act. All clinical notes, digital radio graphs, digital photographs etc remain the property of Dream Smile Dental Clinic. Copies of pictures, radiographs and notes can be made available on request, and Dream Smile Dental Clinic reserves the right to charge an administration fee for these.
